Completed Application Form along with a Demand Draft for Rs.250/- drawn in favour of Father Muller Medical College payable at Mangalore should reach the Admission Officer, Father Muller Medical College, Father Muller Road, Kankanady, Mangalore 575 002 on or before 31.07.2007.

NOTE-On request, we have extended the last date for receiving the completed application forms to 31.07.2007
